Additionally, we may collect personal or sensitive information to assess, investigate, and respond to any instances of abuse, assault, or neglect concerning our customers. This enables us to take appropriate measures to ensure their safety and well-being. Furthermore, we are obligated to provide the requisite reports to the relevant regulatory authorities, including mandatory reporting to the NDIS Quality & Safeguards Commission. Your consent is always sought prior to collecting any personal and/or sensitive information. In certain situations, with your explicit consent, we may also collect personal and/or sensitive information from third-party contractors, agents, or government instrumentalities who are involved in providing our products and services. At iPlan Manager, we prioritize the confidentiality and privacy of your personal information. We do not disclose any identifying information to external agencies, organizations, or individuals unless it falls under the specific circumstances outlined below: • Consent is given for the disclosure to take place. • Disclosure is necessary to mitigate or prevent a serious and immediate threat to an individual's health, safety, or welfare. • Disclosure is necessary to mitigate or prevent a serious (non-imminent) threat to public health, public safety, or public welfare. • Disclosure is required or authorized by law. • Disclosure is reasonably necessary for law enforcement purposes. In cases where verbal disclosure is necessary, it must be made in a private setting. This means that the person making the disclosure must reasonably believe that only the following individuals are present or able to listen to the conversation: • The person making the disclosure. • A lawyer representing the person (if applicable). • One or more individuals to whom disclosure can be made under the Protected Disclosure Act or Regulations. Security of your personal and
sensitive information

The iPlan Manager is committed to maintaining client confidentiality except where the withholding of information would endanger lives or be unlawful. In achieving this, iPlan Manager will, at a minimum, maintain client details on an internal database system. Client information is also stored on secure password-locked databases, except where the client contact only involves simple, quick advice and it is not appropriate to gain their personal details. In these circumstances, separate records recording the number of such contacts over the telephone or at the office are sufficient. Hard copy client files and client documents are not to be removed from the iPlan Manager office unless in exceptional circumstances and with permission from the iPlan Manager management team. In the event of a data breach, where unauthorized loss, use, or disclosure of the personal information we hold occurs, iPlan Manager is committed to following the Data breach notification process outlined by the Office of the Australian Information Commissioner. Our priority is to promptly inform you, enabling you to take necessary steps to mitigate any potential harm or loss, whether financial or otherwise, that may arise from the breach. To ensure the security of your information online, we kindly request your cooperation in adhering to the following security requirements pertaining to the protection of your user identification number and/or password used to access your online account. By following these measures, you contribute to the safeguarding of your personal information: • Keep your user identification number and password confidential. • Avoid sharing your login credentials with others. • Choose a strong and unique password, incorporating a combination of letters, numbers, and symbols. • Regularly update your password to enhance security. • Exercise caution when accessing your account on public or shared devices. • Be vigilant against phishing attempts or suspicious communications and report any such incidents to us immediately. Complaints or concerns
in relation to privacy

If a service user has a privacy-related complaint, we kindly request that it be submitted in writing to iPlan Manager. We are committed to addressing and resolving all complaints in accordance with our Complaints and Feedback policy, ensuring full compliance with applicable privacy regulations.
